What is Form 1095-C?

Form 1095-C provides important information about the health insurance coverage offered to you, as required under the Affordable Care Act, and may be useful for your tax records.

How to Access Form 1095-C

This year we will be using an alternative method to furnish your Form 1095-C, in accordance with the IRS instructions.

Instead of mailing you a paper copy of Form 1095-C, we will make your Form 1095-C available electronically through ADP.
